Read the statements below. You must agree to these terms to activate your SEVIS record.
I agree with the statements below.
STUDENTS are responsible for maintaining their F-1 visa status. Failure to comply with the rules and regulations for F-1 students will result in termination of their SEVIS record/F-1 status. When a student is no longer maintaining status, the student loses all on- and off-campus employment authorization, cannot re-enter the U.S. on the terminated record, and may be subject to investigation by Immigration and Customs Enforcement (ICE).

Students must report
changes
to their personal and academic information directly to International Programs and Services (IPS) within
10 days
.
Students must maintain
full-time status attendance
each term and make
normal progress
toward completion of their academic program as identified on their Form I-20. Dropping or withdrawing from courses resulting in less than a full course load will lead to automatic termination of a student's SEVIS record/F-1 status, unless the student has submitted an “Authorization for Reduced Course Load” form with proper evidence and has been granted permission by the Designated School Official (DSO) to fall below full-time enrollment.
Students must apply for an
extension
of their program end date (as identified on their Form I-20)
prior to
their program end date printed on their Form I-20 in order to avoid termination of their SEVIS record/F-1 status.
Students must obtain permission from IPS before starting an internship, externship, practicum, or any other employment off campus such as
Curricular Practical Training (CPT), Optional Practical Training (OPT
), or “volunteer” work.
Students who voluntarily withdraw from the University or are suspended or dismissed must report to IPS immediately.
Students must leave the U.S.
immediately
when suspended or dismissed from the University, within
15 days
of withdrawal from the University and within
60 days
of completing his/her degree program or OPT employment authorization.
Students with a
terminated
SEVIS record due to a status violation must depart immediately or apply for reinstatement with U.S. Citizenship and Immigration Services (USCIS).

valid passport
